Is there a way to ask a user to provide information when requesting access to a dashboard? This would be helpful for knowing what the user is trying to do, what their role is, how they heard about it, etc.

Currently, there is no available feature through Quick Sight that you can toggle on to setup an information sheet per request, this would need to be done externally.
At AWS, our roadmap is primarily driven by our customers, your feedback helps us build a better service. I’ll go ahead and tag this as a feature request to promote visibility to the support team.
